    • 24/7 Auto page in house social worker p1735 for various issues including: homelessness, discharge planning, sexual assault, crisis/grief, suspected elder/child abuse, recoup care, recently incarcerated, disability benefits (SSI), etc.
    • Homeless Task Force
      • Usually contacted by the social worker on call
      • Available in person during business hours 730a-4p Mon-Fri.

    • Re-entrant Advocacy Community Health (REACH) Community Health Worker helps connect patient's with resources and Fam Med Clinic for primary care
    • 18 years+, released from carceral system in the last 12 months
    • Resources offered: housing, legal, immigration, employment, social security benefits, ID, etc.
    • Option 1: SW consult - "reentry" as indication

    • Can be for DHS or non-DHS (OOP) patients, even if NOT empaneled to Fam Med
