Mycotic aneurysm

Background

A mycotic aneurysm of the pulmonary artery complicating tricuspid infectious endocarditis. The aneurysm has ruptured producing massive hemorrhage.
  • Arterial vessel wall dilation due to bacterial, fungal, or viral infection [1]
  • Incidence is rare
    • 1-5% of aortic aneurysms [2] [3] and intracranial aneurysms [4][5]
    • Seen in up to 2% of infective endocarditis cases [6]

Clinical Features

Signs/Symptoms

  • Fever
  • Pulsatile mass
  • Localized inflammation at arterial access site (if s/p intravascular intervention)
  • Symptoms may vary by location affected (e.g. stroke-like symptoms with intracranial aneurysm, abdominal pain with mycotic AAA)

Labs

Imaging

  • CT angiography (Sensitivity 0.82 (95% CI, 0.77–0.87), specificity 0.93 (95% CI, 0.89–0.95)) [7]
  • MR angiography (Sensitivity 0.79 (95% CI, 0.61–0.91), specificity 0.89 (95% CI, 0.81–0.95)) [8]

Management

  • Emergent vascular surgery consultation
  • Broad spectrum IV antibiotics with gram positive, gram negative, and anaerobic coverage
  • Bilateral large-bore peripheral IVs
  • Definitive care with surgical debridement and endovascular vs open repair

Disposition

  • Admission to OR or ICU in consultation with vascular surgery
