Adult Dosing

Community Acquired Pneumonia

  • 600 mg IV q12h x5-7 days

Cellulitis

  • 600 mg IV q12h x5-14 days

Renal Dosing

  • CrCl 30-50: 400 mg q12h; CrCl 15-30: 300 mg q12h; ESRD/HD: 200 mg q12h

Hepatic Dosing

  • No adjustment
