Adult Dosing

Community Acquired Pneumonia

• 600 mg IV q12h x5-7 days

Cellulitis

• 600 mg IV q12h x5-14 days

Renal Dosing

• CrCl 30-50: 400 mg q12h; CrCl 15-30: 300 mg q12h; ESRD/HD: 200 mg q12h

Hepatic Dosing

• No adjustment

• Allergy to class/drug (see also Cephalosporin Cross-reactivity)
• Caution if hypersensitive to beta-lactams
• Caution if recent antibiotic-associated colitis history

Pharmacology

• Half-life: 2.6 Hours
• Metabolism: Hydrolysis; CYP450: none; prodrug converted to ceftaroline
• Excretion: Urine primarily
• Mechanism of Action: Bactericidal, inhibits cell wall mucopeptide synthesis
