Community Acquired Pneumonia
- 600mg IV q12h x5-7 days
Cellulitis
- 600mg IV q12h x5-14 days
Renal Dosing
- CrCl 30-50: 400mg q12h; CrCl 15-30: 300mg q12h; ESRD/HD: 200mg q12h
Hepatic Dosing
- No adjustment
