Adult Dosing
Opioid-Naive Patients
- Start: 5-15mg PO q4-6h PRN
- Use lowest effective dose and shortest effective duration (NOT a first-line treatment)
- Use caution in older patients
Opioid-Experienced Patients
- Individualize based on current opioid intake q4-6h PRN
- Use caution in older patients

- 0.05-0.15mg/kg PO q4-6h PRN
