Pediatric Dosing

GERD: 5-10 mg/kg/day PO BID

    • Adult: CrCl < 50: 150 mg Qday; CrCl < 10: 75 -150 mg Qday; HD: give dose after dialysis
    • Pediatric: CrCl < 10-50: decrease dose 50%; CrCl< 10: decrease dose 75%; HD: Give dose after dialysis
