- For appointments during the Day can often call the clinic (x8252, 5141/49) or page the on call RN (p9995, only on call until 8pm)
- Coumadin Clinic Form not always necessary if you can make a live appointment, otherwise should be filled out and can be faxed to the clinic directly.
