Endophthalmitis

Background

  • Inflammation (usually infectious) of the aqueous or vitreous humor
  • Frequently leads to loss of vision (ocular emergency)

Causes

  1. Cataract surgery
  2. Penetrating trauma
  3. Extension of keratitis
  4. Hematogenous spread

Clinical Features

  1. Headache
  2. Eye pain
  3. Photophobia
  4. Vision loss
  5. Ocular discharge
  6. May also see:
    1. Conjunctival/scleral injection
    2. Chemosis
    3. Hypopyon
    4. Uveitis

Treatment

  1. Emergent ophtho consult (for intravitreal abx)
  2. Vancomycin 1gm IV q12hr + ceftazidime 3gm IV q8hr

Disposition

  • Admit
