Adult Dosing

General

• PO
  • Immediate-release
    • Regular strength: 650 mg q4-6 hours
    • Extra strength: 1000 mg every 6 hours
  • Extended-release: 1300 mg every 8 hours
• Rectal: 650 mg every 4 to 6 hours
• IV:
  • <50 kg: 15 mg/kg every 6 hours or 12.5 mg/kg every 4 hours  ; maximum single dose: 15 mg/kg/dose (≤750 mg/dose)
  • ≥50 kg: 650 mg every 4 hours or 1000 mg every 6 hours
• Maximum daily dose: 4g

Pediatric Dosing

• 15 mg/kg/dose Q6 hours
