  1. Sudden onset or accelerating pattern
  2. No similar headache in past
  3. Age >50yr
  4. Occipitonuchal HA
  5. Visual disturbances
  6. Exertional
  7. Family history of SAH
  8. Focal neurologic signs
  9. Diastolic BP > 120
  10. Papilledema
  11. Jaw claudication

Clinical Context Headache in setting of:

  1. Infection
  2. Cancer
  3. Immunosuppression
  4. Syncope
  5. Trauma
  6. Altered mental status
  7. Systemic illness (fever, stiff neck, rash)
  8. Nausea/vomiting
  9. Patient on anticoagulation, steroids, NSAIDs
