Call the TSICU Attending for any of the following conditions:

  1. Acute hypoxemic or hypercarbic respiratory failure NOT responsive to conventional therapies
  2. Unplanned extubation requiring possible reintubation
  3. Hemodynamic instability:
* SBP <90 mmHg and requiring initiation or escalation of vasopressors
* SBP >180 mmHg OR DBP >120 mmHg AND end-organ dysfunction
* patients who are non-responders (>2L crystalloids AND ongoing evidence of organ dysfunction
  1. Massive hemorrhage (bleeding associated with hemodynamic instability) due to ANY cause (includes, but not limited to, epistaxis, GI bleed, airway/trachea, vascular access site)
  2. Activation of Massive Transfusion Protocol (MTP)
  3. Refractory intracranial pressures (ICPs)
  4. Acute decrease in GCS OR encephalopathy of new onset without identifiable etiology
  5. Status epilepticus or seizures refractory to initial therapy
  6. Recommendation(s) or orders from consult services which have a direct impact on patient care
  7. Radiologic findings relayed from radiologist to team verbally
