General Guidelines
- Trauma/SICU Attending approval is required for all transfers and downgrades
- Trauma patients
- Reconcile orders BEFORE patient leaves the unit
- Trauma patients
- Non-trauma patients
- Primary service is responsible for reconciling meds once downgraded and transferred out of the TSICU
- Non-trauma patients
- Out of network patients will require a minimum of 24 hours of observation in a non-ICU monitored bed PRIOR to transfer
- ICU-to-ICU transfers require approval by BOTH service Attendings
- Contact Laura Santana for all ICU-to-ICU transfers
- Sign out patient verbally using IPASS (page appropriate team) and sign transfer summary BEFORE patient leaves the unit
- Always remember to sign out patient’s baseline GCS/mental status and systems-based plan
